New Employee Forms

In the mail you will receive your offer letter, two copies of your job description, an authorization form for your pre-employment physical and a copy of your union contract, if applicable.

Please sign your offer letter and return it to the Human Resources office.  Below are required forms, processes and information for you.  Each item that needs action will be indicated with an asterisk * .  Please complete all of the necessary forms and return them to the Human Resources office before or on your start date.  Thank you.

Sign and return each acknowledgment form.

  • Personnel Rules and Regulations
  • Exposure Control Plan
  • Electronic Messages Record Retention Policy
  • Security Awareness Testing & Training Policy
  • Use of Town Vehicle Policy (include a copy of your drivers' license)

 

Your to do list!

  • Contact CorpCare to schedule your pre-employment physical. Remember to bring your authorization form and a copy of your job description to your appointment (included with your offer letter)
  • Complete the process for Criminal History Request and schedule your appointment for fingerprinting (click here for instructions)
